本校は、教育活動を推進するにあたり、個人情報の取扱いに関して、関係法令等を遵守するとともに、関係者の皆様への信頼を保持するため、本方針を定めます。

## １ 関係法令の遵守と方針の周知

本校は、「個人情報の保護に関する法律」、「長野県個人情報保護条例」等関係法令を遵守し、個人情報の適正な取扱いに努めます。また、この方針を本校ホームページに掲載する等、その周知をはかります。

## ２ 個人情報の範囲について

個人情報とは、個人に関する情報であって、当該情報に含まれる氏名、生年月日その他の記述等により特定の個人を識別することができるものを指します。

本方針では、本校関係者の個人情報の取扱いをその対象としますが、本校関係者とは、

（1）本校在籍生徒等（本校志願者を含む）  
（2）本校保護者等（生徒の家族も含む）  
（3）本校卒業生等（中退等で在籍していた者も含む）  
（4）本校教育関係者等（学校評議員、企業・地域等の本校協力者、班活動後援会関係者、本校への来校者、本校への電話やメール等の送信者）  
（5）本校教職員等（教職員の家族も含む）

を指すものとします。

## ３ 個人情報の収集及び利用について

個人情報の収集は原則としてその個人の同意のもとに行われるものであり、本校ではその利用目的を明示した上で必要な範囲の情報を収集し、その目的に沿って個人情報を利用します。なお、上記２にいう本校関係者の区分並びにその関係者の個人情報に関わる主たる利用目的は以下のとおりです。

## ４ 個人情報の提供及び委託について

（1）取得した個人情報については、以下の場合を除き、第三者に提供することはしません。  
① 転校の場合や取調べに応じる場合など法令に基づく場合  
② 医療機関での緊急治療の場合など、人の生命、身体または財産の保護のために必要だが本人の同意を得るのが困難な場合  
③ 犯罪被害者となるおそれがある場合、また、生徒の安全を確保し非行・犯罪を予防するために特に必要だが本人の同意を得るのが困難な場合

（2）個人情報を第三者機関に委託して活用する場合には、予め委託先機関を明示して個人情報を収集すると共に、委託先機関の安全管理についての確認に万全を期します。

## ５ 開示、訂正請求等について

本方針が遵守されていないと思われる個人情報について、また、個人情報の開示、追加、訂正、削除あるいは利用停止等について、個人情報の本人から、合理的な理由及び手続きに基づいて要請があった場合には、これに適切に対処します。

## ６ 指針の改善措置について

個人情報の取扱いに関して、法令や社会情勢等の変化に的確に対応するように努め、また、必要に応じて本指針の変更、修正、追加等の改善措置を実施します。

## ７ 個人情報の取扱い窓口について

個人情報の管理に関して、意見、質問等がある場合には、下記あてに、連絡又は問い合わせてください。
